To evaluate the effectiveness of clear aligners in ortho-surgical preparation, as well as to report the advantages and disadvantages of using clear aligners for orthognathic surgeries. Narrative-type literature review. The search for articles was carried out digitally, in the Virtual Health Library, Scientific Electronic Library Online and Google Scholar databases. Articles in Portuguese, English and French that were published in the last 10 years and that were available in full text were included. Traditionally, ortho-surgical preparations have been performed with conventional fixed appliances, but with the growth of digital flow orthodontics, the use of invisible aligners has been described as an effective alternative. Among the advantages highlighted by the aligners, the authors highlighted greater adherence to treatment due to better aesthetics, comfort and maintenance of good oral hygiene. one of the main disadvantages of this technique. The use of orthodontic aligners was reported with excellent employability and effectiveness in ortho-surgical preparation. Despite preliminarily demonstrating good results, the literature on the subject still lacks studies with larger samples and longer follow-up times.
