Agenesis of the maxillary lateral incisors, a prevalent congenital anomaly, impacts aesthetics and function, requiring individualized therapeutic approaches. This literature review explores three management options: (1) space maintenance for rehabilitation with fixed prostheses or implants in the lateral incisor region; (2) complete space closure by mesialization and reanatomization; (3) mesialization with prosthetic rehabilitation in the canine region. The methodology involved analyzing articles published between 2000 and 2025, selected via PubMed. The third option is illustrated by a clinical case of a 15-year-old patient with agenesis of the maxillary lateral incisors. Mesialization of the canines, with space for prostheses in the canines, was chosen due to socioeconomic limitations. Results indicate aesthetic and functional efficacy, highlighting mesialization as accessible, particularly with orthodontic mini-implants, which optimize tooth movement. The review underscores the importance of early diagnosis and multidisciplinary collaboration for optimized rehabilitation in young patients.
